How to make image clear using photoshop cs4?

how do i edit my image on photoshop cs4 to look clear yet blured or something like this one-

Add a layer above the current layer with the picture you want to fade...fill the layer with white( the new layer you just added...) change the opacity to near zero for that layer...i set mine to 15%


and then applied a slight Gaussian blur.... to 0.4 or something like that.
